Decline: Name on invoice does not match name on submitted claim

If you have been declined for this reason, it is because the first and last name on the rebate application form does not match the first and last name on the invoice you had provided. We are required to provide the rebate to the person listed on the invoice. If there has been an error made on your invoice, please see your dealer and have a proper invoice printed. If you have submitted the claim on behalf of another individual, please re-submit the claim if you are still within the date range, and use their information instead of your own.
