My claim has been placed on hold for missing patient information?

It looks like during processing, we were unable to determine the patient/customer information on your receipt. All rebate claims must provide an itemized receipt listing:

  • Purchase location
  • Purchase Date
  • Contact Lenses purchased
  • Number of boxes
  • Patient Information – First Name, Last Name, Phone number, Address, City, State and Zip Code

Please ensure your documentation clearly indicates each of these pieces of information.
