My claim has been placed on hold for missing patient information?
It looks like during processing, we were unable to determine the patient/customer information on your receipt. All rebate claims must provide an itemized receipt listing:
- Purchase location
- Purchase Date
- Contact Lenses purchased
- Number of boxes
- Patient Information – First Name, Last Name, Phone number, Address, City, State and Zip Code
Please ensure your documentation clearly indicates each of these pieces of information.
