My claim has been placed on hold for name not on invoice or receipt?
It looks like during process we were not able to verify your purchase details. Both the sales receipt and eye exam/fitting receipt must identify the patient information including the name and/or address. To provide updated documentation to ensure that all information is clear, please use the ‘Check Existing Rebate” option. You will be asked to supply your claim number and email address. Please ensure you select “Update Claim” once you have completed your updates.
