On Hold: Quantity submitted does not match quantity on the invoice

  • Contact lenses purchased
  • Number of boxes
  • Patient Information - – First Name, Last Name, Phone number, Address, City, State and Zip Code


To provide updated documents, please return to the rebate website coopervisionpromotions.com and select the "Check Existing Rebate" option. You will be asked to supply your claim number and the email address on the application. Once your claim has been located, you may add additional documentation via upload. Please ensure you select "Update Claim" once completed.
