What information is required for a Delivery/Installation rebate?

Delivery /Installation rebates require the entry of the model you purchased in addition to the delivery/installation cost in order to validate the rebate claim. Please select your model from the pre-populated list, provide your serial number and cost of the item, next please select delivery cost or installation as your model from the pre-populated list, provide your cost of delivery/installation.

In addition, delivery/installation fees must be shown on the purchasing invoice. If a delivery/installation invoice is submitted separately, it must reference the product you have purchased.
