My claim has been placed on hold for a Health Care Provider not listed on receipt, what do I need?

It looks like during processing we were not able to verify the Health Care Provider. The treatment receipt must identify the Health Care Provider. To provide updated documentation please use the "Check Existing Rebate” option. You will be asked to supply your claim number and email address. Please ensure you select “Update Claim” once you have completed your updates.
